Here's how it works: book your at-home draw with OptiVena first, then order your tests through Sonora Quest's My Lab ReQuest program. We collect your sample and route it directly to the lab. We serve Queen Creek, Gilbert, Chandler, Mesa, Scottsdale, Tempe, Phoenix, and surrounding areas.
Prices are set by Sonora Quest and may change. All tests are self-pay -- insurance cannot be billed for self-ordered testing per Arizona law.

Complete Blood Count with Differential (CBC)
Evaluates red blood cells, white blood cells, and platelets. Helps screen for infections, anemia, inflammation, and a range of other conditions. No fasting required -- water and medications are fine before your draw.
Comprehensive Metabolic Panel (CMP)
14-test panel assessing liver and kidney function, electrolytes, blood sugar, and blood proteins. A broad picture of your organ health and metabolic balance. Fast for 10-12 hours before your draw.
Heart Health
Cardio Health Screen (Lipid Panel)
Measures LDL (bad) cholesterol, HDL (good) cholesterol, and triglycerides. Used to evaluate heart and artery health and assess cardiovascular risk. Fast for 9-12 hours before your draw.
Diabetes & Blood Sugar
Hemoglobin A1c
The gold standard for monitoring blood sugar control over the past 2-3 months. Commonly used for diabetes diagnosis and ongoing management. No fasting required.
Thyroid & Hormones
Thyroid Screen (TSH, High Sensitivity)
Screens for thyroid disorders that can affect weight, energy, mood, and overall health. Identifies both overactive (hyperthyroidism) and underactive (hypothyroidism) thyroid function. No fasting required.
