We do not service Maricopa or Casa Grande, or any areas north of Anthem or south of Coolidge and Florence, including Tucson, Eloy, Arizona City, or other Southern or Northern Arizona locations. If your city is not listed below, we are unable to schedule an appointment in your area.
- OptiVena is a self-pay service. We do not bill or accept insurance for any of our fees. All major debit and credit cards, plus HSA and FSA cards, are accepted.
- For routine and STAT appointments only, we collect your insurance information so that the testing laboratory can bill your insurance for the cost of the testing itself, separate from our mobile collection fee.
- Pricing is subject to change at any time without prior notice. Rates displayed reflect current pricing.
Draws take place at a residence, workplace, or hotel. We do not perform draws in public spaces such as parking lots, parks, or other open meeting points. If a home or workplace visit isn't an option, please provide another private, clean space, such as a hotel room, rather than a public meeting point.
