• OptiVena is a self-pay service. We do not bill or accept insurance for any of our fees. All major debit and credit cards, plus HSA and FSA cards, are accepted.
• For routine and STAT appointments only, we collect your insurance information to bill the testing lab directly for the cost of the testing itself, separate from our self-pay collection fee.
• Pricing is subject to change at any time without prior notice. Rates displayed reflect current pricing.

Draws take place at a residence, workplace, or hotel. We do not perform draws in public spaces such as parking lots, parks, or other open meeting points. If a home or workplace visit isn't an option, please provide another private, clean space, such as a hotel room, rather than a public meeting point.
