For Healthcare Providers

Phlebotomy Services for Providers

When a patient can't easily get to a draw site, the labs you ordered tend to get delayed or skipped. Our certified phlebotomists collect at the patient's home or workplace anywhere in the Phoenix Valley and deliver the specimens to the lab on your order. Here is how your office can book.

Three Ways Providers Work With Us

Pick the option that matches how often your office sends patients and how you prefer to handle payment.

Available

Book Online for a Patient

Best for a single patient or an occasional referral. Your office books through our online scheduler and pays the collection fee at checkout.

Available by request

Office-Billed Booking

Best for offices that refer patients on an ongoing basis. Your office schedules the visits and we invoice your office directly.

At capacity

Priority Provider Account

Includes shortened arrival windows and exact appointment times. We are not adding new priority accounts at this time.

Book Online for a Patient

No setup and no account needed. Your office can book a patient in a few minutes.

STEP 1

Choose the City and Service

On our booking page, select the city where the patient will be seen. The services and exact pricing for that area appear right away.

STEP 2

Enter the Patient's Details

Fill out the appointment with the patient's name, contact information, and visit address, then upload the lab order. Lab orders are due right after the appointment is booked.

STEP 3

Pay at Checkout

The collection fee is paid by card when you book. Arrival windows are 2 hours for routine lab work, specialty kits, and SneakPeek, and 1 hour for STAT and therapeutic phlebotomy.

Office-Billed Booking

For offices that send patients to us regularly, we can invoice your office instead of collecting payment at each booking. Your office schedules the visit, sets the details, and remains the point of contact for the patient. We reach out to the patient only when our phlebotomist arrives.

Lab orders are due right after the appointment is booked, the same as for any other booking.

Office-billed appointments use the same arrival windows as online booking: 2 hours for routine lab work, specialty kits, and SneakPeek, and 1 hour for STAT and therapeutic phlebotomy. This option is set up at our discretion, based on referral volume and scheduling availability.

To request office-billed booking, call 623-692-0527, text 480-256-2858, or email scheduling@optivena.com.

Priority Provider Accounts

Priority accounts include shortened arrival windows and exact appointment times for your patients. We are currently at capacity and are not adding new priority accounts. Online booking and office-billed booking are both available to your office in the meantime.

Draws at Your Office

We can see an individual patient at your office. Book it the same way as any other visit and use your office address as the visit location.

We are not offering on-site support for group draws or clinic days at this time.

Clinical Trial Draws

We collect for clinical trials and research studies on a per-patient basis, at the participant's home. Each visit is scheduled with our standard 2-hour arrival window. Group draws are not available for studies at this time.

  • Pricing is a custom quote, based on the study's requirements and any supplies we need to provide, such as shipping materials
  • Shipping labels must come from the ordering facility and be sent with the lab order. We do not provide shipping labels
  • Lab orders are due right after the appointment is booked
  • Each participant is booked as an individual home visit

To request a quote, call 623-692-0527, text 480-256-2858, or email scheduling@optivena.com with the study's collection and shipping requirements.

What We Need From Your Office

Lab orders are due right after the appointment is booked. This applies to every booking, whether a patient schedules directly or your office books for them. Having the complete order up front lets us confirm every requirement and prepare supplies ahead of time and prevents appointment delays, cancellations, and redraws.

Appointments without a confirmed order on file are automatically cancelled, and your office is welcome to rebook once the order is ready.

OptiVena delivers provider-ordered specimens to Labcorp or Sonora Quest
  • An official lab order with patient information, tests ordered, and diagnosis codes
  • Your office's contact details, lab account number, and the provider's signature
  • The order uploaded at booking, or faxed to 623-288-5074 right after the appointment is booked
  • STAT marked on the order itself if the lab should run the tests STAT
  • The patient's insurance information, if the lab will be billing insurance for testing
  • For specialty kits, the kit and all paperwork in the patient's hands before the visit

Therapeutic Phlebotomy Orders

Therapeutic phlebotomy appointments require a signed requisition from the ordering provider. Orders can be written as a standing order for up to 12 months. Please include:

  • Patient information and demographics
  • Volume of blood to be removed at each appointment
  • Frequency of treatment
  • Any hemoglobin or hematocrit minimum parameters
  • Ordering provider's information

Provider FAQs

Can my office book and pay for a patient's visit?

Yes. Use our online scheduler, enter the patient's details, and pay the collection fee at checkout. Offices that refer patients regularly can also request office-billed booking and receive an invoice instead.

What is the difference between office-billed booking and a priority provider account?

Office-billed booking changes how the visit is paid for: we invoice your office. A priority provider account adds shortened arrival windows and exact appointment times. Priority accounts are currently at capacity.

What arrival window should we give our patients?

Routine lab work, specialty kit collections, and SneakPeek have a 2-hour arrival window. STAT and therapeutic phlebotomy have a 1-hour arrival window. These apply to both online and office-billed appointments.

Do you bill the patient's insurance?

Our collection fee is self-pay and is not billed to insurance. The testing is separate: the lab bills the patient's insurance for the tests when insurance information is provided.

What does a visit cost?

Collection fees depend on the service and the patient's city. The booking page shows the exact price once a city is selected, and the full list is on our pricing page.

Which areas do you cover?

We serve the greater Phoenix Valley, from Anthem in the north to Coolidge and Florence in the south. The booking page lists every city we schedule in.

Can you come to our office for a group of patients?

Not at this time. We can see an individual patient at your office, but we are not offering on-site support for group draws or clinic days.

How do we send you the lab order?

Upload it when you book, or fax it to 623-288-5074 right after the appointment is booked. If your office faxes the order, please call 623-692-0527 or text 480-256-2858 to confirm we received it. Appointments without a confirmed order are automatically cancelled.
